HeartSine Samaritan AED Recalled at Costco Over Component Failure
What Happened
On July 31, 2026, Costco Canada posted two urgent medical device recall notices from Stryker covering certain HeartSine Samaritan PAD SAM 350P, SAM 360P, and SAM 500P automated external defibrillators (AEDs) sold on Costco.ca.
The main recall concerns the membrane tail, an internal component. Stryker identified an issue that could cause an intermittent fault at any point when the device is holding a charge in preparation to deliver therapy, while delivering a shock, or after shock delivery. The device becomes inoperable after the failure occurs. Stryker's testing indicates the likelihood of failure is very low, and there have been no reported adverse events related to this issue. Health Canada classifies the membrane tail recall as a Type II recall (RA-82380).

How to Identify the Product

Check the front label of your AED for the model name — SAM 350P, SAM 360P, or SAM 500P — printed below "HeartSine samaritan PAD". Only certain devices are affected: the recall covers specific serial numbers (Stryker field actions PFA 3977961, PFA 4280335, PFA 4257283). What Should You Do
Stryker's guidance is different from a typical recall — do not discard your AED:
- Complete and return the recall acknowledgement to support@aedrecall.ca by August 31, 2026.
- Continue using your AED if you do not have another public access defibrillator available. Per Stryker, the likelihood of failure is very low, and having an AED available remains safer than having none.
- Download and keep the maintenance supplement with your AED user manual until your replacement device arrives.
- Share this recall information with anyone responsible for your AED and keep it with the device until it has been replaced.
Once your acknowledgement has been received and replacement units become available, Stryker will contact you to arrange shipment of your replacement AED.
